Hello....
My all Academic documents(LC; marksheets ; graduation certificate) shows AKSHAY VINOD AKBARI

Whereas my goverment proof( PAN; Aadhar; passport etc) shows AKSHAY VINUBHAI AKBARI

My father real name is VINUBHAI and in his all record( Academic+ goverment) his name is recorded as VINUBHAI.....In short no mistake in their documents...

I asked number of legal advisors but all gave me different answer......My question is is there any requirement of gazette? How many gazette I need to do.....

You can start by making an affidavit stating that "AKSHAY VINOD AKBARI" and "AKSHAY VINUBHAI AKBARI" belong to the same person, and the difference in the middle name is due to an error. This affidavit needs to be notarized or you should approach the educational institutions where you obtained your academic documents and request them to update your records accordingly.

2) IDENTITY PROOF : Pan card or Passport or Driving Licence or Aadhar Card or Voters Card (ANY ONE)
3) ADDRESS PROOF : Ration Card or Aadhar Card or Electricity Bill (ANY ONE)
4) AGE PROOF : Birth Certificate, School/ College / TC leaving Certificate, (INCASE OF MINOR BONAFIDE CERTIFICATE with SCHOOL IDENTITY CARD)
5) AFFIDAVIT : ( If Any Made for any reasons like Numerology, Astrology, Marriage, Passport, Adoptions etc )
6) APPLICATION FORM : Duly Signed Application Form with OLD & NEW SIGNATURE other details like Old & New Name, NEW Correspondence address, Contact Details & reason for Name change
7) CASTE CERTIFICATE (IF ANY) : SC/ST/NT1,2,3

To resolve the issue related to the correction of the name that appears differently in different documents legally, you need to pursue the following steps. 1) An affidavit is a legal document that is used to state facts under oath.
