Fake information furnished in Annexure C for Minor Passport

My wife applied for my minor son's passport by publishing false information about me. In Annexure 'C', she mentioned that I am out of India and cannot be available at the time of passport application. She also forged by placing her signature in my passport Xerox copy and submitted the application. I do not have the passport copy of the baby.
How serious is this offence? Whom should I report it to if needed? What is the punishment for this offence?
Please advise.

It is an offence under the Passports Act 1967 to furnish false information in the application.
You can contact the nearest Regional Passport Office to report incident regarding the suspected false information in your son’s passport in person. If you cannot contact the nearest RPO, you can report the issue at your nearest police station.

You should report this offence to the nearest police station or the Regional Passport Office. Section 12(1) of the Passport Act, 1967, makes providing false information for passport application an offence punishable with imprisonment for a term which may extend to two years, or with fine which may extend to five thousand rupees, or with both. Additionally, the forgery of a signature is punishable under Section 465 of the Indian Penal Code, which provides for imprisonment of up to two years, or with fine, or both.
