Name inclusion in birth certificate

I want to include my son's name in his birth certificate. He is 6yrs old. He is being homeschooled and we plan to homeschool him till his 10th. BBMP wants school certificate for name inclusion. How do we proceed?

A.Dear Client,
A Birth Certificate Update Form can be obtained from the municipal corporation office(BBMP) or gramme panchayat where your child was born to add your name to the birth certificate. Obtain an affidavit from a nearby notary, then submit the update form to the concerned officer designated as Registrar under the Birth and Death Registraion Act, 1969 about adding your name to the birth certificate of your son.
