Hello! I am a resident of Bengaluru, Karnataka but born in Deolali Nasik, Maharastra. My Birth Certificate miss-spelled my name but all other documents have a different name. Is it possible to amend or update my birth certificate to reflect the name I use everywhere else? Also my birth certificate doesn't have a registration number.

A.Dear Client,
The Birth Certificate is issued by the Registrar under the Registration of Birth and Death Act, 1969 attached to the Civic Authority/Municipality/Panchayat based on the documents required under the Act like the hospital discharge report issued in respect of newborn baby mentioning name of his or her biological parents. The documents required under RBD Act in a State for issuing a birth certificate of an individual are as follows: Birth Certificate issued by the Hospital or a registered medical practitioner, Proof relating to the birth and age - e.g, the birth certificate, matriculation certificate, PAN Card, AADHAAR Card, Voter ID Card, etc. and if required, an affidavit specifying the place, date and time of birth sworn by the applicant who needs correction in the Birth Certificate. So, you need to apply to the concerned authority of your birthplace i.e, Nasik, Maharashtra for the desired correction in the Birth Certificate and for the issue of one birth certificate afresh registered under the Registration of Birth and Death Act, 1969. and based on that birth certificate you can seek correction in all other documents

A.Dear client,
Submit an Affidavit of name change in your respective district court (Eg. Mayo Hall in Bangalore) before a Magistrate or Notary Public. In the affidavit, mention the current name and the desired new name. Also mention in brief the reason for name change. The affidavit must be made on a Non-Judicial stamp paper of minimal value (not more than Rs.10 or Rs.20)
It is preferable to use the assistance of an advocate to prepare the affidavit in a legally appropriate format.
Also submit attested proof of Indian citizenship along with the affidavit. The document becomes valid/ recognised if signed by the Magistrate or Notary.
Publish announcement of name change in two local newspapers- one in the local official language and another in English. This is required by law, to invite objections (only if any) to the name change, allowing them an opportunity to take it up with the authorities.
Publish an advertisement of the name change in the Official Gazette of your respective state. This is also called Gazette Notification. This can be done by approaching the Government Press of Karnataka and submit form for the same, furnishing details such as old and new name, address etc.
Pay the prescribed fees.
Your name change notification will be published in the state’s Official Gazette. Copies of the same will be sent to the address given by you
