A.Dear client,

First, you will need to work with a local notary to draft an affidavit for a name change. You will have to explain your reasons for wanting to alter your name. You should receive notification of its publication in your state's official gazette after it is published. Filling out the deed changing name form available at the gazette office will do this. This is the stage where the Transport Secretary and the Department of Commerce will check your paperwork. To do this, you must send
A letter requesting permission to change your name should be sent to the secretary. The alteration of the name form. Two copies or more of the local publication where your name change was announced. A.Dear Client,
For correction of your Date of Birth in your academic records, you need to apply to the Registrar under the Birth and Death Registration Act, 1969 who is available in the office of Municipal Corporation or the office of the Dist. Magistrate along with all the relevant and supporting documents including an affidavit sworn before a first-class Judicial Magistrate and the copy of gazette notification publishing correction of DOB based on the contents of the affidavit.
